![]() If you can’t see your target, you attack at disadvantage. In practice, it really doesn't change much about the combat. Your target must take the hide action if he wants to avoid location – and then if he attacks, his location is revealed until he hides again. ![]() If you knew where he was before, you still know where he is. So this takes care of locating your target. It is basically a perception check against an opposed hide roll. My DM limits this based on distance from target and other ambient noise, but note that there is no stated range listed in the DMG. Otherwise, anyone in the area who knew their location before the spell was cast, still knows it after the spell was cast, even if the person in the fog moves. So, creatures fighting in a fog cloud, or darkness, must take the hide action if they wish to conceal their location. To be hidden, you must be not seen and not heard. Not seen and not seen and not heard are two different things. Now, common sense would tell us that if you can’t see your target, then you must not know where he is. Seeing out of a fog cloud is equally impossible. A creature effectively suffers from the blinded condition when trying to see something in that area. This means that vision is blocked entirely.
